A CIBC Investor's Edge Investment application form must be completed for any of the following account types. Please note that all forms as listed below are required to open your account, unless otherwise specified. Other additional documents might be required in order to open your account.


Individual/Joint Investment Application (Online or PDF )

*For a Joint account, each applicant must provide approved ID or a W8BEN/W9


Corporate or Partnership

*Articles of Incorporation and a Certificate of Corporate Status also required


Nominee Account

*For informal "family nominee accounts" each child must provide approved ID or W8/W9


Partnership

*For Partnership accounts approved ID or W8-BEN for each partner and a w-IMY for the Partnership is required


Sole Proprietorship

*Registration Paper also required

*An unregistered Sole Proprietor account must provide only approved ID or W8/W9


Formal Trust

*Trust Documents are also required (Legal Trust)


Investment Club

*Approved ID or W8-BEN for each member of the Investment Club

*For formal (legally documented) Investment Clubs a w-IMY is also required


Estate

*Notarized Copy of the Death Certificate (certified true copy also accepted in Quebec) is also required

*Notarized copy of Probated Will (certified true copy also accepted in Quebec) is also required

*Letter of Direction signed by the executor is also required

*Declaration of Transmission (Quebec only)
